A funded Home Medicines Review requires a referral from an eligible medical practitioner, most commonly the person’s GP, and the funded review must be provided at no charge to the patient. What happens after you press send

An eligible practitioner decides Most referrals come from the person’s GP; certain specialists can also refer. The practitioner confirms clinical need.
Planned home visit After both launch approvals and an eligible referral, the planned interview would be at home for 45–60 minutes, in English, Urdu or Hindi.
Written report The report goes to the referrer and the patient’s GP if different. Prescribing decisions remain with the patient and their doctors.
