Planned Home Medicines Review coverage across Melbourne’s north

HomeMed’s planned service area starts from a Craigieburn base and includes Roxburgh Park, Meadow Heights, Greenvale, Mickleham, Kalkallo, Epping, Wollert, Broadmeadows and nearby northern suburbs, with wider metro enquiries considered by arrangement. Funded referrals and reviews will begin only after the MMR credential and service-provider approval are confirmed. A funded HMR requires an eligible medical practitioner referral and must be provided at no charge to the patient; a separate referrer appointment may have its usual fee unless bulk billed.
