The interview is planned for the patient’s home. With the patient’s consent, a family member may join and help provide relevant information. The pharmacist prepares a written report for the referrer and, when different, the patient’s GP.

Planned interviews are available in English, Urdu or Hindi. Other language-support needs can be discussed before an appointment.

The planned interview length is 45 to 60 minutes. The pharmacist would discuss the medicines the patient reports using — prescriptions, non-prescription products, vitamins and medicines added by a specialist — and how each is actually taken. The patient can raise practical questions, and a family member may join when the patient wants them there.

How it works, from your side

1. Get in touch. Call, email or use the online form and tell me what you’ve noticed. An enquiry does not commit you to a review.
2. Eligible referral. Most referrals come from the person’s GP; specified specialists can also refer.
3. Home interview and report. Once the service opens, the interview is planned for the patient’s home; family may join when the patient wants them there. The report goes to the referrer and, when different, the regular GP.
